All children regardless of age, including newborns and infants, must have their own passport

All minor’s must apply in person with both parents/guardian(s)

All minor’s must provide his/her Social Security number

Requirements.

1. Application

To apply for a new passport you need to complete a new passport application form DS-11 (But do not sign it yet).

2. Photos

Valid U.S. passport.

3. Proof of Citizenship

Copy of your travel itinerary, or copy of your airline ticket.

4. Evidence of Relationship (One of the following):

• Minor’s certified U.S. birth certificate with both parents’ names
• minor’s certified Foreign Birth Certificate with both parents’ names
• minor’s Report of Birth Abroad with both parents’ names
• Adoption Decree with adopting parents’ names
• Court Order establishing custody
• Court Order establishing guardianship
• If only one parent present, the second parent must submit notarized Statement of Consent (Form DS-3053
• If only one parent available, the applying parent must submit Form DS-3053 stating why the non-applying parent/guardian’s consent cannot be obtained or submit minor’s certified U.S. or foreign birth certificate listing only the applying parent.
• Court order granting sole custody to the applying parent.
• Adoption decree (if applying parents is sole adopting parent).
• Death certificate of non-applying parent

5. Travel Itinerary

Copy of your travel itinerary, or copy of your airline ticket.

6. Parents or Guardian Proof of Identity

Proof of identity. (A copy of your state I.D. or a driver license).

7. Authorization letter

Release letter. (Authorization form).

8. Government Fees

1. Make two checks or money order for:
   • $ 170.00 (minors 16 years and older) payable to U.S. Dept. of State
   • $ 140.00 (minors 15 years and under) payable to U.S. Dept. of State
   • $ 25.00 payable to the clerk of the acceptance agency.
2. Take all the above documents to the passport acceptance agency and sign the application in front of the clerk.
3. Take the sealed envelope from the clerk and call our office for further instruction.
