Name Change or Correction to a current Passport
You need to apply for a name correction
If your name has changed, and your passport was issued less than one year ago.
If your passport was issued more than one year ago you need to renew your passport and complete form DS- 82.
Requirements.
1. Application
To change your name in your passport you need to complete form DS- 5504.
2. Passport
Valid U.S. passport.
2. Travel Itinerary
Copy of your travel itinerary, or copy of your airline ticket.
4. Proof of Name change
A marriage license, a court order, and a copy of your state I.D. or a driver license).
5. Authorization letter
Release letter. (Authorization form).
6. Government Fees
a. Make two checks or money order for:
$ 60.00 payable to U.S. Dept. of State..b. Take all the above documents and call our office for further instruction.
