If you need to add visa pages to your currently valid U.S. passport complete form DS- 4085

Requirements

1. Application

To add a visa pages on you current passport your need to complete form DS-4085.

2. Passport

A valid U.S. passport

3. Travel Itinerary

Copy of your travel itinerary, or copy of your airline ticket.

4. Authorization letter

Release letter. (Authorization form).

5. Government Fees

Make a checks or money order for:
$ 142.00 payable to U.S. Dept. of State.

Take all the above documents and call our office (480) 894-8810 for further instruction.

Remarks:

Fedex $ 22.00 each way for overnight delivery.

7. Form of payment

We accept credit card, money order, cashiers check, company check or cash.
