Lost or stolen passport
If your still valid passport was lost or stolen, please submit the form DS-64 with the form DS-11 application when you apply for replacement.
Passport reported lost or stolen are invalidated and can no longer be used for travel.
Requirements
1. Application
To replace a lost or stolen passport you need to complete form DS-64 and form DS-11
2. Photos
Two identical photos, 2×2.
3. Proof of Citizenship
An original or certified copy of your birth certificate, an original certificate of naturalization, or a previously issued passport.
4. Travel Itinerary
Copy of your travel itinerary, or copy of your airline ticket.
5. Proof of Identity
Proof of identity. (A copy of your state I.D. or a driver license).
6. Authorization letter
Release letter. (Authorization form).
7. Government Fees
- Make two checks or money order for:
- $ 170.00 (adults 16 years and older) payable to U.S. Dept. of State
- $ 140.00 (minors 15 years and under) payable to U.S. Dept. of State
- $ 25.00 payable to the clerk of the acceptance agency.
- Take all the above documents to the passport acceptance agency and sign the application in front of the clerk.
- Take the sealed envelope from the clerk and call our office for further instruction
